Also, I will say that Doctors like to point to Pharma and Insurance as the problem. Pharma likes to point to Insurance and Hospitals as the problem. Insurance likes to point to Hospitals and Pharma as the problem.
I think the assumption is that the cost of healthcare is probably high because of the combination of all of them. They all have powerful lobbying arms. I would also assume that each of them has a vested interest in the other becoming less powerful... And the US probably would get a lot of value out of all of them becoming less powerful. Probably something along the lines of Single Payer Healthcare would help a lot...
