The middle-men making the profits have no incentive to disclose this -- my guess it's the for-profit insurers optimizing what they bring in versus actually spending money on healthcare...
It's an interesting problem for sure, but until voters get the will to abolish the current system by voting for changes in laws and regulations, I don't see any of that changing anytime soon. The lobbies will never allow it...
