Ask HN: Data Modeling Tools?

What tools do you use to perform your data modeling tasks, and why?

Specifically, I'd love to find a tool with:

  - A nice, clean interface
  - Ability to easily convert a data model into SQL CREATE TABLE statements (supporting multiple syntaxes)
  - Is relatively affordable

Anything out there?
